✅ Mandatory requirement: All participants must complete an initial individual assessment before joining the group program. This ensures your exercises are safe, appropriate, and tailored to your goals and presentation.
We are pleased to offer specialised exercise physiology sessions for individuals with type 2 diabetes. Our goal is to support you in managing your condition through personalized exercise and education sessions.
Eligibility
To qualify for our exercise physiology sessions, you must:
Be over 18 years of age.
Have type 2 diabetes.
Be referred by your GP using the "Referral form for Group Allied Health Services under Medicare for patients with type 2 diabetes."
Your GP will need to prepare a new GP Management Plan (item 721 or 732) for your referral.
What to Expect
Initial Assessment:
Your journey begins with an initial assessment, which will be conducted under Medicare code (item 81100 - in person, 93284 -telehealth).
Exercise and Education Sessions:
After your initial assessment, you will progress to your 8 funded exercise and diabetes education sessions (item number 81115).
Both these sessions will be bulk billed under Medicare, meaning there is no out-of-pocket cost to you.
Important Note
Please note that these sessions are distinct from the "Referral Form for Individual Allied Health Services under Medicare for patients with a chronic medical condition and complex care needs." This is a care plan where you can access various other services (up to 5 per year). However, we are also happy to see patients under this referral too.
