Mast Cell Activation Syndrome (MCAS) can involve multi-system symptoms that fluctuate and may be triggered by stress, heat, illness, exertion, or environmental exposures. At Palms Physiotherapy & Allied Health, we provide symptom-aware allied health support focused on pacing, function, and safe participation—working alongside your GP/immunology/allergy team.

Mast cells are part of the immune system and release chemical mediators (such as histamine) as part of normal immune responses. In MCAS, mast cells may release mediators inappropriately or excessively, which can contribute to a broad range of symptoms across different body systems.
Symptoms and triggers vary significantly between individuals. MCAS diagnosis and medical management must be led by your GP and/or specialist team (e.g., immunology/allergy). Our role is rehabilitation support—helping you manage functional impacts and build safe routines.
MCAS can present in many ways. Some people experience:
Skin symptoms: flushing, itching, hives, swelling
Respiratory symptoms: chest tightness, wheeze, shortness of breath (in some)
Gastrointestinal symptoms: nausea, abdominal pain, diarrhoea, reflux
Cardiovascular symptoms: palpitations, light-headedness, dizziness
Neurological symptoms: fatigue, “brain fog,” headache, sensory sensitivity
Exercise or heat intolerance in some individuals
Because these symptoms overlap with other conditions, medical assessment is important.

Occupational therapy can support day-to-day function when symptoms and fatigue disrupt routines.
OT may assist with:
Energy conservation and pacing strategies
Task modification for home, work, and self-care routines
Planning tools for variable days (good days vs flare days)
Environmental strategies to reduce overload and improve predictability
Equipment recommendations where clinically indicated
Speech pathology is not routinely required for MCAS. It may be relevant if you experience:
Swallowing concerns (dysphagia) that require assessment
Voice issues (e.g., throat tightness/strain) affecting communication
Communication participation issues linked to fatigue or cognitive load
If these concerns are present, assessment determines the appropriate pathway and whether medical referral is needed.
Find the right support by discipline, including physiotherapy, occupational therapy, speech therapy, exercise physiology and other allied health services.
Because MCAS triggers vary, we take a cautious, individualised approach. In sessions, we can:
Plan around known triggers (heat, strong scents, exertion intensity, time of day)
Use gradual progressions and frequent check-ins
Build a “flare plan” so you know how to modify activity safely
Encourage coordination with your GP/specialist recommendations (hydration, medication timing, emergency plans)
Please tell us about: known triggers, medication timing, and any anaphylaxis action plan.
Seek urgent medical attention if you experience:
Signs of anaphylaxis (difficulty breathing, swelling of tongue/throat, collapse, severe dizziness)
Severe chest pain, fainting, or rapidly worsening shortness of breath
Severe allergic reaction symptoms that escalate quickly
Follow your medical team’s emergency plan and call emergency services when needed.
Living with MCAS can be challenging and unpredictable. We can support families and carers with:
