At Palms Physiotherapy & Allied Health, we provide allied health services for older adults across Perth, with appointments available in clinic and, where suitable, through mobile home visits.Â
We support seniors who want to stay safe, mobile, independent and well-supported at home, in the community, or when transitioning between hospital, home and ongoing care.
Our services may be suitable for older people accessing care through Support at Home, private health insurance, private fee-paying arrangements, selected Medicare GP chronic condition management referrals, and for eligible DVA clients, depending on the service required and the relevant funding rules.Â
Funding pathways and item eligibility vary, and final approval always rests with the relevant program or funder.
We understand that every older person’s situation is different. Some clients prefer to attend one of our clinics in Malaga or Noranda, while others may benefit from support in their own home.
Depending on clinical needs, goals and therapist availability, we may provide:

Our aged care and senior support services are focused on function, safety, mobility, independence and participation.Â
We work with older adults who may be experiencing changes related to ageing, illness, surgery, injury, falls risk, reduced strength, changing mobility, communication needs, swallowing concerns, or difficulty managing everyday tasks at home.
We can assist with:
maintaining mobility and confidence

strength, endurance and general conditioning
pain-aware movement and function

Our physiotherapists can support older adults with mobility, balance, strength, transfers, falls prevention, pain-related movement limitations, post-hospital recovery, and general physical function. Physiotherapy may be helpful for seniors who want to stay active, improve confidence with walking, or maintain independence at home.
Exercise physiology may assist older adults who need structured, individualised support to improve physical conditioning, strength, endurance, and activity tolerance. Programs are tailored to the person’s presentation, goals, function and health considerations.
Occupational therapy focuses on helping older adults manage everyday activities more safely and independently. This can include support with self-care, home routines, fatigue management, transfers, environmental setup, equipment recommendations, and practical strategies to make daily life easier.
Speech pathology may support older adults who are experiencing communication changes, cognitive-communication concerns, voice changes, or swallowing and mealtime difficulties. Input is based on individual need and clinical assessment.

We can provide home-based assessments to identify barriers to safety, mobility and independence. This may include reviewing access, transfers, bathroom setup, bedroom layout, mobility within the home, and practical considerations for daily living.
Where clinically appropriate, we can assess and make recommendations for home modifications intended to improve accessibility, function and safety.Â
Depending on the person’s needs, this may include recommendations relating to bathroom access, rails, ramps, step access, circulation space, or other environmental changes.Â
Recommendations are based on assessment findings, clinical reasoning and the relevant funding pathway.Â
For DVA clients, RAP can cover eligible home modifications where there is a clinically assessed need and approval requirements are met.
We can complete functional and capacity-focused assessments that help clarify how a person is managing mobility, transfers, self-care, daily activities and safety within their home environment. These assessments may help inform service planning, support recommendations and equipment decisions.
We can assist with the assessment and prescription of suitable mobility aids and daily living equipment, including trials and practical recommendations where appropriate. Depending on the client’s needs, this may include support relating to:
walking aids
wheelchairs and mobility devices
transfer aids
bathroom and toileting equipment
seating and positioning supports
selected activities of daily living aids
For eligible DVA clients, the Rehabilitation Appliances Program provides aids, equipment and modifications to support safety and independence, including mobility devices and adaptive household items where clinically indicated.
Support at Home is the Australian Government’s in-home aged care program for eligible older people. It replaced the Home Care Packages Program on 1 November 2025 and is designed to help older people remain at home for longer with assessed supports and services. Access generally starts through My Aged Care and an assessment process.
Some services may be claimable through private health insurance depending on your level of cover, the discipline involved and your insurer’s rules. We recommend checking directly with your fund.
Some clients may be eligible for a referral from their GP under a GP chronic condition management plan for allied health services. For eligible patients, Medicare can provide benefits for up to 5 individual allied health services per calendar year, depending on referral and claiming requirements.
Eligible DVA clients may be able to access certain aids, equipment and home modifications through the Rehabilitation Appliances Program (RAP), based on assessed clinical need and the relevant prescribing requirements.
We also welcome private clients who want direct access to allied health support for mobility, home safety, equipment, rehabilitation, communication, daily living, or healthy ageing.
