Pelvic pain can be exhausting. It can affect sitting, walking, exercise, sleep, toileting, and intimacy — and for many people it comes and goes in cycles or flares. At Palms Physiotherapy & Allied Health, we provide pelvic health support that focuses on function, comfort, and confidence—and we work alongside your GP and specialist care when needed.
Our role is to help you understand what may be contributing to symptoms, reduce sensitivity where possible, and build practical strategies that fit your day-to-day life.
Quick note on diagnosis
Pelvic pain has many possible causes. Pelvic health physiotherapy does not replace medical assessment. If you haven’t yet discussed symptoms with your GP, we recommend doing so—especially if pain is new, worsening, or associated with other concerning symptoms.
Pelvic pain is pain felt in the lower abdomen, pelvis, vulva/vagina, rectum, tailbone, hips or lower back region. It may be constant or intermittent, sharp or aching, and can be influenced by movement, bladder/bowel habits, hormones, stress, previous injury or surgery, and muscle and nerve sensitivity.
Pelvic pain can present in many ways, including:
pain with sitting, walking, bending, or certain positions
pain with bladder filling, urgency, or after toileting
bowel discomfort, constipation, or pain with bowel motions
pain with intercourse, pelvic examinations, tampon use, or penetration
lower abdominal “dragging” or heaviness
flare-ups after stress, poor sleep, illness, or activity spikes
And it often impacts:
confidence leaving the house
exercise and fitness routines
work tolerance and concentration
relationships and intimacy
mood, sleep, and overall quality of life
Pelvic pain is usually multi-factor. Contributing factors can include:
pelvic floor muscles that are overactive (tight/guarding) or poorly coordinated
sensitivity of pelvic nerves and pain pathways
bladder/bowel habits (urgency patterns, constipation, straining)
hormonal changes (cycle, postpartum, peri/menopause)
scars or tissue sensitivity after surgery, childbirth, or injury
endometriosis or other medical conditions (requires medical diagnosis)
stress, trauma history, or persistent pain patterns that increase nervous system “alarm”
We take a respectful, non-judgemental approach and focus on what’s relevant for you.
Your plan is tailored, and may include:
education (what’s happening, why flares occur, what helps)
pelvic floor strategies (often relaxation/coordination first; strengthening only when appropriate)
breathing and pressure management to reduce pelvic loading
bladder/bowel habit strategies (urgency tools, constipation management support)
graded return to activity so you can move without provoking flare cycles
flare planning (what to do in the first 24–72 hours, and how to progress)
sensory and pain modulation strategies (practical tools to reduce sensitivity)
coordination with your GP/specialist if medical review is needed
We’ll start with:
a detailed history and goal setting (what matters most to you)
discussion of symptoms, triggers, and what improves/worsens things
an assessment appropriate to your comfort and consent
a clear plan with next steps you can start immediately
Pelvic health appointments are always consent-based. You can ask questions, pause, or decline any component.
See your GP promptly if symptoms are new, worsening, associated with bleeding, fever, urinary burning, or significant bowel changes.
Seek urgent care for severe pain with fever, inability to pass urine, heavy bleeding, fainting, new leg weakness/numbness, or rapidly worsening symptoms.
